FC: Car 15, Assistant Chief Galvin, reports: There is a top floor concrete form that has collapsed and struck the crane, the crane has not collapsed. We have 1 DOA, 1 injured removed to street level being looked over by EMS. Buildings Dept. is currently assessing the structure. There is loose debris on the roof that is being secured. Probably will hold, all units assigned to the box are being held at this time.

FC: Car 15 reports Primaries complete and negative, no more threat of collapse, still in process with EMS and Buildings of identifying victims removed to hospital.

FC: Car 15 reports Secondaries are complete and negative with the exception of a 20x20 section on the NE corner on the 39th and 40th floors that was involved in the collapse; a visual search of that area was conducted due to condition of the building. Under Control.

Car 15: All fire department operations have concluded, the building is now being looked over by the Buildings Dept. and the construction company, PD is investigating the streets and adjoining structures. We had 1 DOA and 3 injured, transported by EMS. All FD units will go 10-8 as they become available. The construction company notified us that all employees have been accounted for, secondaries are complete.
