How and/or why did E238/HL106 miss this job? ???

Companies were operating on Manhole fire at Nassau Ave & Monitor St which is about 6 blocks away. Then calls came in for Nassau & Van Dam for possible extension. E-229 E-259 TL-146 were sent to check it out.

With millions of barrells of oil and gas leaked into the ground of Greenpoint since the 1900's,it wasn't a far reach to guess it could have been extension of a manhole fire. Which has happened before on multiple occasions.
