Miracle, faith, whatever word would like to call it. Has to be. Your right, that corner should of been packed at that time.

Its the vault or cover that pops when road salt and water get in there.  THe conduit starts to burn, gas and pressure build up under the lids causing them to pop off, fire and brimstone eminate from the hole.  Its not TNT or a hollywood explosion, dangerous? yes, deadly?, can be, but not as bad as you think.  I've been within 10 feet of them as they have gone off, you get peppered with debris, its loud as hell...and the lid can hit and kill you, but on the whole, you live....but your skivvies won't survive.  Manhole explosions are a common thing in NYC, especially this time of year.

No, vaults are where the bigger components are.  Manholes have the service feeds.  Think of it like this.....vaults are underground, electric substations, (like the ones found in most towns that are fenced in), manholes contain service points for the grids that supply the buildings.....much like service poles and their drops.  Usually a fire in one will affect the other.  A major concern for these incidents is fire to extend into the basements, or where ever the main breaker box is in the building....which fire ould also run throughout the building.....
