Chief Carlock died of smoke inhalation while operating at a two-alarm fire.

Deputy Chief Bernard Carlock, Divison 4, lost his life at a 2nd alarm fire in a vacant tenament at 7-13 E 106th Street after being overcome by smoke. He was treated by Dr. Archer at the fire location and transported to 5th Avenue Hospital, where he passed away. The vacant tenement was the scene of several previous fires.

Chief Carlock had almost been killed years earlier on February 27, 1907 at a 2nd alarm basement fire in a store at 573 8th Avenue. Foreman (Captain) Carlock, Engine 26 - 1st due engine, was advancing a line to the back of the store and then down stairs into the basement with two Engine 26 members, FF Adam Damm and FF Harry Baker. They were overcome by ammonia fumes and smoke when they reached the basement. Foreman Carlock tried to carry FF Damm out of the basement but was overcome and lost consciousness. FF Damm and FF Baker lost their lives but Foreman Carlock was rescued by Ladder 4 and recovered.
