Eerily similar to the line-of-duty death of FF Harold Hoey of Ladder 17.
May both Harold and William rest in peace.

I'm surprised that more emphasis wasn't put on the one thing that would've prevented the accident. Most departments and I believe FDNY also require that anyone in an elevated bucket be securely tethered to the bucket. That fact alone would prevented this death. Obviously a very sad situation however. I wonder if training will provide more emphasis on the fact that you need to be tethered to the bucket?
