  I found an old assignment card from 1959 with numerous updates up to the "War Years". The letter "D" after the box number means that it was a DRB (discretioary response box):   
 
I have to disagree there were no DRBs back at that time when cards were used........i think it meant "dummy box"....the R next to ENG*80 indicated that the 3rd due was "in reserve" since  a SQUAD was assigned on the 1st alarm.....the reserve ENG would not go even up to a Fifth but rather they would be available for subsequent alarms in the area....some ENGs missed a lot of work that way.
 
The third closest Engine not responding was known as the "Reserve Engine" the idea at that time was not to completely strip an area...(even though relocations were still done)........i know of one particular BKLYN  Engine who was in reserve on all the boxes in a several block area that was being vacated & demolished to make way for several Hi Rise Project type bldgs..........there was a tremendous amount of jobs there during about a year that it took to vacate the people & business s & level the area......at the time demo companies got premium pay if working in a fire damaged bldg.......so do the math.......this particular Engine could look out the top floor rear window of the FH & see the smoke ........even if it was a multiple they did not go. 
 
 
Chief, you are correct about the "D" it stood for  "Dummy" box which meant that a pedestal or nothing was at the particular corner but was assigned a Box number.Manhattan was unique in that Lenox Ave and Seventh Aves had a Box Number for all boxes from 110St. to I believe 145th St. but only had the working heads on every other box. The Dummy boxes were only transmitted if a phone alarm was received and the address showed that it was a "Dummy" Box location. tough to explain but I think you get the drift.
The Engine you are referring to might it have been 230 ? or 237?
 
JOR correct on the dummy box & correct on 237 when Lindsay Park Houses were being built.
