1/29/2025 Washington DC Plane in the Water

Almost exactly 24 hours prior to the DCA mid-air collision, Republic Airlines flight Brickyard 4514 was forced to abort an approach into DCA Runway 19 flying the Potomac visual near Georgetown. This was due to a TCAS alert for a northbound Blackhawk (PAT 11) on Route 1 at a prohibited 300-600 feet despite both aware of the other.

I just heard this on TMZ, they we're interviewing a person who I believe was a former army pilot.

Critical 1 in risk analysis is defined as any system or subsystem that, if compromised, will result in the loss of the Space Shuttle and crew.

Mullane didn't mention that the Space Shuttle had 861 critical 1 items, of which the O rings were only one

The launch decision considerations were far more complicated than described.

NASA did have data and guidelines to justify the launch with the well known O ring issue'

As I recall Mullane is a West Point graduate. He should know that one of the burdens of decision makers is having to make an arbitrary choice based on never enough information and then having to stand by it.

Everyone is a genius in retrospect.

If you want to criticize a wrong. high-stakes decision, you will have to get in line.
 
It probably won't be verified until the helo is raised from the river; but could the crew have been wearing night vision appliances, and if so, are they monochromatic? Could this cause any confusion by aircraft lights blending with street lights etc. through those goggles, making the aircraft difficult to identify?
