Wasn't there a shipboard fire awhile back where the Navy had to write off a submarine? I believe it was a case of arson from a disgruntled sailor.

This was all easily predictable and the nation is paying the price.
After two USN destroyers had collisions at sea a couple of years ago, a commission studied the causes. Not Good. As reviewed in the Wall Street Journal, sailors were buried under Mickey Mouse discipline, officers seemed concerned about good press and keeping superiors happy. Diversity training took precedence over seamanship and warfighting.
Don't think this isn't happening elsewhere. To quote a veteran chief friend of mine (ex-USAR; WTC, Katrina, etc.) "I may not know the latest firefighting tactics, but I can do an EEOC complaint paperwork in my sleep".
