Maybe someone looking to cover it up. On a less morbid note possibly something electrical that went un-noticed after the occupant died.

Is this a normally 109 or 149 first due box? Did 109 beat the Castle in?

It depends on who turns out quicker when it comes to that area. Could’ve easily been one or the other
 
long run for both trucks however as dob said 109 has a straight run down 3rd ave and have least amount of turns
(3rd ave express)
